Documentation Guidelines. This document lists the specific details required by the Alabama Community College System to document disabilities. This information is also included in the ADA Student Handbook.
Request for Documentation. This form may be used for your convenience to request your documentation be sent to the Wallace State ADA Office.
Student Accommodations Request. Each semester students needing accommodations for their classes must fill out this request form and submit to the ADA Office.
